Survey questions to ask sisters about visiting teaching

Hi there.I was called a few months back to visiting teahing leader. We seem to have many sister who dont go out to do the watch care,i am wanting to put a simple survey together to ask what holds them back.not those words exactly though.

I know peoples lives are busy,mine is thats for sure.But how do i ask things with out offending?  I am wanting a few some questions to ask in a confidential survey,that they write down and pass to me,so then we can look at what the issues are and then address them in another meeting.

It was interesting when talking to a friend,about this,she commented to me,gave me an example for there are many working sisters and do work long hours and stuggle to do anything,bit the comment to me was.  If some one was offered $10000.000. they didnt have to do anything for it,didnt have to pay it back,didnt have to qualify to recieve it,theyjust had to drive 2 hours to get it,then they would make the time,find the time to go get the money.Yet we often dont make the time to serve to look after those in our care.

This is so true.

Ive love to some help with questions to ask in the survey so sisters tell me what holds them back and perhaps,get the true vision on Visiting Teaching/watch care with may be some of the questions in the survey

I am just wanting about 8 questions to be able to help them and see what are the problems they face.

Do you think you can help me. I need this bt the mid of may.
