Primary Assignment Sheet

I am looking for an Assignment Sheet to give to our Primary Teachers to Assign all the Opening Stuff (Prayer, Scripture, Theme, Talks and Article Of Faith) I found a link that was linking me to this site but now I can’t find one anywhere, do You know where I can find something like that? I found one that had a Detachable top for the Presidency, and then the bottom portion was for the Class as reminders. Am I dreaming that I found something like that? I would love any help you could give me! Thanks!
