Enrichment Night Service Auction

Hi,
I need some help locating or coming up with some questions and the points for them to use for our service auction. We had one last year and everyone had a great time, but none of us saved a copy of the questionaire that we used. So as RS secretary, I’m now searching high and low for a quetionaire that we can use. Any help would be greatly appreciated.
